A GP Chronic Condition Management Plan (GPCCMP) is what your GP uses to refer you to allied health services - like our Exercise Physiologists and Physiotherapist - with a Medicare rebate. It replaced the old GP Management Plan and Team Care Arrangement system, so if you remember needing multiple appointments and several other providers involved, that's no longer the case.

Book a longer appointment with your GP
Ask specifically for a GP Chronic Condition Management Plan (GPCCMP) appointment - this needs more time than a standard consult, so let the reception team know when booking.

Your GP assesses and prepares your plan
Your GP determines whether your condition qualifies (generally, a chronic condition that's been or will be present for 6 months or more) and prepares your GPCCMP.

Ask your GP to refer you to Movement in Mind
Your GP writes a referral letter directing you to us for Exercise Physiology and/or Physiotherapy - just let them know which of our practitioners or services you'd like to see. This referral is valid for 12 months from the date of your referral.

Bring your referral to your first appointment
Bring your referral letter and Medicare card when you book with us. We're Medicare bulk-billing eligible for GPCCMP services - our team will confirm exactly what applies to your plan.
What a GPCCMP covers
Up to 5 sessions a year
Most patients can access up to 5 individual allied health sessions per calendar year under a GPCCMP (up to 10 for patients of Aboriginal or Torres Strait Islander descent).
Split however you need
Your 5 sessions can go entirely to one discipline or be split — for example, 3 sessions with an Exercise Physiologist and 2 with our Physiotherapist.
Group sessions for type 2 diabetes
If your GP assesses you as suitable, you may also access up to 8 group Exercise Physiology, dietetics, or diabetes education sessions per year - this can apply to our Group Exercise Physiology classes.
